This plan cannot be combined with any other special offer or discount.
Your policy will renew automatically each year. If your premiums are not paid, then your dental plan benefits will become null and void.

Annual Premium
There’s no ID card, no group or member number. All of your membership information will be kept on file with our office. Your effective date is the day you sign up and your renewal date is the same date every year.

The Rudnick Dentistry Dental Plan is an annual reduced fee dental plan that allows individuals and families, just like yours, to receive dental services from our office at prices to better fit today’s economy.
How?
Submit your premium payment, and you’re effective immediately to start receiving benefits and discounts for your dental needs. That’s it! Very SIMPLE!
Who?
Anyone who is a patient of Rudnick Dentistry.
When?
Benefits begin IMMEDIATELY! Members must remain on the plan for a minimum of 12 months.

Program Limitations and Exclusions
- This plan cannot be combined with any other special offer or discount.
- When third party financing is used, the amount of the discount will decrease by 10%.
- Orthodontic, TMJ services and full mouth reconstruction surgery are not part of this dental plan.
- Prophy cleanings by our office are defined as the removal of coronal plaque, calculus buildup, and stains on the tooth above the gum line. Deeper cleanings fall under a periodontal category.
- Loss or theft of dentures, crowns, and bridges are not covered.
- This plan is only good at Rudnick Dentistry. If you are referred to a specialist, they will not offer the discount.
- Procedures performed in the hospital, hospitalization, or hospital charges of any kind are not covered.
- Yearly benefits are not carried over to the next year.
- Rudnick Dentistry Dental Plan benefits cannot be transferred over to other family members.
- Annual membership dues are non-refundable.
- No refunds are given if a patient chooses not to use their dental plan.
- This is a dental discount plan and is not dental insurance.
- The dental plan cannot be used in conjunction with a dental insurance policy.
- Costs of dental care covered under automobile medical are not included.
- All payments are due at time of service to receive a discount. Any services rendered that are not paid for at the time of service will be billed at the usual, higher fee.
- Your policy renews automatically each year. If premiums are not paid, then you will not qualify for any discounts.
